Practical Support
Practical Support
Our hospice team work closely with patients and those people important to them in many different ways. We can quickly access support and practical advice as living with a life-limiting illness has an impact on every area of life. We engage holistically with the wider hospice team and agencies to ensure ongoing care is individualised.
Patients who are supported in the community rather than in the inpatient unit may also benefit from support and advice. Hospice services help clients and their families navigate planning for end of life care, understanding their treatment plan and be vocal about their needs be they physical, emotional, spiritual, familial or financial. They can help overcome crisis situations and connect to other support services.
Social Care
The Clinical Support team aims to support patients’ families, friends and carers in many different ways.
Spiritual and Religious Care
Living with a life-limiting illness affects not only the body but also the mind and spirit.
Advance Care Planning
Care for people at the end of their lives may be complex and often involves input from many different health and social care professionals working within the NHS and independent sector.
Family support
Support may be needed throughout the course of a life-limiting illness. Our dedicated team ensures that care is given not only to patients but also to those who care for them.
St Helens and Knowsley Citizens Charter for End of Life

Digital Legacy
For many people these days a number of aspects of our lives are conducted online and we use our electronic devices for everything from capturing video and photographs to banking and shopping.
